What is Venous LEAK

A venous leak (more accurately called veno-occlusive dysfunction) is a condition in which blood enters the penis during an erection but leaks out through the veins too quickly to maintain a firm erection.

Symptoms

  • Difficulty maintaining an erection despite adequate sexual stimulation
  • Erections that become soft during intercourse
  • Better erections when lying down than standing (sometimes)
  • Normal sexual desire and orgasm may still be present

Causes

Venous leak can be associated with:

  • Aging
  • Diabetes
  • High blood pressure
  • Smoking
  • Pelvic trauma or surgery
  • Structural changes in penile tissue (e.g., fibrosis)
  • Sometimes no clear cause is identified

Diagnosis

A urologist, especially one specializing in sexual medicine, may use:

  • Medical history and physical examination
  • Blood tests (hormones, diabetes screening, etc.)
  • Penile Doppler ultrasound after injection of a medication that induces an erection
  • Other specialized vascular tests in selected cases

Treatment Options

  • Oral medications such as PDE5 inhibitors (e.g., Sildenafil or Tadalafil)
  • Vacuum erection devices
  • Penile injection therapy
  • Lifestyle changes (smoking cessation, exercise, management of diabetes and cardiovascular risk factors)
  • Penile implant surgery for severe cases that do not respond to other treatments

Important Note

 

Many men who believe they have a venous leak actually have erectile dysfunction from other causes (vascular disease, anxiety, hormonal issues, medication side effects, etc.). A proper evaluation by a urologist is important before concluding that a venous leak is present.
