What is Venous LEAK
A venous leak (more accurately called veno-occlusive dysfunction) is a condition in which blood enters the penis during an erection but leaks out through the veins too quickly to maintain a firm erection.
Symptoms
- Difficulty maintaining an erection despite adequate sexual stimulation
- Erections that become soft during intercourse
- Better erections when lying down than standing (sometimes)
- Normal sexual desire and orgasm may still be present
Causes
Venous leak can be associated with:
- Aging
- Diabetes
- High blood pressure
- Smoking
- Pelvic trauma or surgery
- Structural changes in penile tissue (e.g., fibrosis)
- Sometimes no clear cause is identified
Diagnosis
A urologist, especially one specializing in sexual medicine, may use:
- Medical history and physical examination
- Blood tests (hormones, diabetes screening, etc.)
- Penile Doppler ultrasound after injection of a medication that induces an erection
- Other specialized vascular tests in selected cases
Treatment Options
- Oral medications such as PDE5 inhibitors (e.g., Sildenafil or Tadalafil)
- Vacuum erection devices
- Penile injection therapy
- Lifestyle changes (smoking cessation, exercise, management of diabetes and cardiovascular risk factors)
- Penile implant surgery for severe cases that do not respond to other treatments
Important Note
Many men who believe they have a venous leak actually have erectile dysfunction from other causes (vascular disease, anxiety, hormonal issues, medication side effects, etc.). A proper evaluation by a urologist is important before concluding that a venous leak is present.
